Who is the Older Person’s Star for?
The Older Person’s Star was developed to help older people with re-enablement and measure progress towards maximising independence and wellbeing. It is suitable for use in settings with one to one support, but also includes a shorter version for use in resource centres or day services where there is less one to one time with older people.
The Older Person’s Star helps older people and those supporting them identify progress towards key outcomes like staying as well as you can, feeling positive, being treated with dignity and managing money, and give clarity on where to focus next.
- Are there any other Stars available for older people?
The Independent Living Star has been developed for any adult who is receiving support to stay living in their home for as long as possible. It is appropriate for older people, people with physical disabilities and others needing additional support to be independent. It is a variant of the Older Person’s Star.
Using the Outcomes Star approach
The Older Person's Star is designed to be used as part of the Outcomes Star approach. That’s why we provide essential training and ongoing support to all licenced practitioners, so you can embed the Outcomes Star approach in your practice.
How does the Older Person’s Star work?
The Older Person’s Star covers seven key outcome areas that help support older people to maximise their independence and wellbeing, and assesses their progress towards each on a five-stage Journey of Change.
You and the people you support will fill out the Star together, and use it to better identify where their strengths lie and where they might need more help. And to make plans for moving forward along the journey.
