How Outcomes Star can support and strengthen your Family Help transformation
Embedding prevention, outcomes, and relational practice across local support for children and families
Local Authorities across England are working to deliver multiple policy priorities – from the transformational agenda of the Family First Partnership programme and the shift to Family Help, through to the continued rollout of Family Hubs and delivering to ambition set out in the Best Start for Life vision.

Supporting the shift to Family Help
The Family Help reforms aim to create a more coherent, whole‑family system, bringing together Targeted Early Help and Children In Need support into a single continuum.
At the heart of this shift is the Single Family Help Assessment and Plan – a shared, multi-agency framework led by a Lead Practitioner and designed to reduce duplication, strengthen relationships and improve support. Outcomes Star can be complement safeguarding practice as part of the Single Assessment and Plan in a number of ways.
For this blended approach to be successful in practice, practitioners must be able to complement their safeguarding practice with meaningful engagement of families with many different needs and circumstances, and the building of intrinsic strengths in the people they support, such as resilience, identity and self-belief.
Supporting relational practice
Family Help emphasises relational, strengths-based practice. The Outcomes Star is built around collaborative conversations with children, young people and parents and carers, helping practitioners to:
- build trust, engagement and motivation – making change visible for all
- explore strengths as well as challenges
- co-produce goals and plans.
This aligns closely with the whole-family, relationship-based model at the heart of reforms.
Revealing underlying factors
Traditional assessments are effective at capturing external factors, such as housing or school attendance, and safeguarding tools focus on current and future risks by design.
However, what can be harder to explore and capture is the internal, individual experience underneath the surface. Family Star Plus and My Star, for example, cover areas such as how a parent sees themselves and sees their parenting, and a child’s confidence, identity and emotional well-being.
This can help practitioners identify or understand early signs of fragility that may not yet be visible in external behaviours or known by individuals themselves.
A shared framework across services
Outcomes Stars provide a common language of outcomes, easily understood in plain English and simple ‘I Statements.’ Because Stars travel easily, they enable practitioners from different disciplines (social care, health, education, voluntary sector) to understand and track family progress consistently. This is critical for Family Help, where multi-agency teams must work as one system rather than separate services.
Strengthening Early Help and Family Hubs
Family Hubs are the place-based delivery model for integrated early help. They bring together services such as health visiting, parenting support, SEND services, and early years provision to provide a single front door for families.
Outcomes Star enhances the effectiveness of this model in several ways.
Creating a joined-up experience for families
In a hub model, families often interact with multiple professionals. Outcomes Star provides a shared, easy-to-understand tool that can travel with the family, reducing duplication so that conversations build on each other and families don’t have to “repeat their story.”
Supporting integration across services
Family Hubs depend on strong connections between services. Outcomes Stars acts as an integration tool by:
- aligning different services around common outcomes
- supporting joint planning and review
- helping professionals understand their contribution to shared goals.
Enabling whole-family insight
Family Hubs are designed to support families from pregnancy through adolescence (0–19/25). The Outcomes Star approach allows services to consistently:

- respond in a coordinated, holistic way – capturing supporting evidence for step up/down decisions.
Strengthening outcomes focus
Early Help services are increasingly required to demonstrate impact. Outcomes Star provides:

Delivering the Best Start for life vision
The Best Start for Life policy focuses on improving outcomes in the critical 1,001 days (pregnancy to age 2), with an emphasis on:

- confidence for practitioners to work in a consistent, evidence-informed way.
